Welcome to the 24-25 school year! We are very excited about the possibilities due to the merging of the former 4j transition programs CLP and Connections. We strive to serve all students in the district who are eligible and will create an individualized schedule for each student. September 2024
To all 4j Community Transition Program (“4j CTP”) Students & Families,
Hello! We hope you have had a safe and enjoyable summer, and are as excited as we are to start the new school year! Transition services will be offered through the 4J Community Transition Program. Our goal is to help students between the ages of 18-21 with the transition from High School to independent living. High school graduates with an alternative certificate, modified diploma or a GED, and who have received Special Education Services are eligible for this program. Instruction takes place in several locations within the community, depending on student need. Students may take part in small group seminars, vocational work crews and/or individual advocate sessions designed to support each student’s goals.
The CTP staff will host an informational meeting on:
● Wednesday, September 4th at 6:30pm in person at the Connections building at 2120 Bailey Hill Rd, Eugene OR 97403
The first two days of school will be Thursday, September 5 and Friday September 6th at 2120 Bailey hill road.
We will have get-to-know-you and orientation activities. Orientation will cover routines, safety procedures and opportunities in the communities around the CTP campuses. There will be time for questions and social time to get to know the new students and staff. Students will work on schedules according to their individual plans. The CTP staff will do our best to ease this transition as we create a program that serves each student’s needs in an enriching environment. The program will be familiar and refreshing at the same time.
Students will begin work at their job sites and classes on Monday, September 9.
